The Holden Town Clerk's Office has been designated as a Passport Acceptance Facility for the acceptance of completed passport applications. This office does not issue passports or trace the same.
First Passports:
• |
Completed Application Form DSP-11 (available from this office, post offices or downloaded from the U.S. Department of State web site, www.travel.state.gov). Do not sign the application before appearing at the Town Clerk's Office. |

Birth certificate - certified copy containing the name of the applicant, date of birth, place of birth, file or registration date and certification (seal). |
• |
Identification - acceptable documentary evidence of identity (driver's license). |
• |
Two (2) passport photos - identical, 2" x 2", full face, plain light background. |
• |
Appropriate Fees - two checks - one in the amount of $75.00 payable to "U.S. Department of State" (if a minor age 15 and under the fee is $60.00), and a second check in the amount of $25.00 payable to "Town of Holden". |
Renewals:
• |
You are qualified for this if the old passport was issued within the past 15 years from the Date of Issuance. |
• |
You were at least 16 years old when your most recent passport was issued. |
• |
You can submit that most recent passport. |
• |
You are using the same name or can submit the marriage certificate or court order to show the change of name. |
• |
If you fulfill all of the above, submit an Application for a Passport By Mail, Form DSP-82, along with two new photos, your old passport and $75.00 by check or money order to the address listed on the reverse of the application form. |
General Information:
1. |
Normal processing is 8 weeks from date of receipt. |
2. |
Expedited service is available if passport is needed within 4 weeks. There is an additional fee of $60.00 per application for expedited applications. |
3. |
If you have lost your passport you must apply for a new one, not a renewal. |
4. |
Only one person may be on a passport. Each child must have his or her own passport. |
5. |
Passports are valid for 10 years for an adult (18 and over) and 5 years for a minor. |
6. |
Both parents must appear and sign for a child under the age of 16. For a minor aged 16-17, one parent must appear and cosign the application with the minor. |

Traveling Within 14 Days:

You must call the National Passport Information Center at 877.487.2778 for an appointment. Use a touch-tone phone 24 hours a day to schedule an appointment. |
• |
Appointments may only be scheduled if you are traveling within 14 days and have proof of travel (tickets or travel itineraries) and if your travel requires a foreign visa. |
